All patients must complete a CONSENT, ASSIGNMENT, RELEASE FORM at each visit.

Workers Compensation patients being treated for a new injury must also complete a Health History Form and Workers Comp Registration Form.

Urgent Care patients must complete the above mentioned consent, assignment, release form and the Urgent Care Symptom Report. If you have not completed an Urgent Care Patient Form or a Health History Form for our clinic within the last year, please also complete those.
